This form is to be completed by an adult attending Sikh2Inspire Camp 2023 and any children under the age of 15 (including babies) that are attending with him/her. All other adults will be required to complete a separate form. PLEASE NOTE - This does not guarantee a family room

Room allocation preference - please state the name, age & email address of the person you would like to be allocated a room with. Rooms will be allocated based on gender e.g. female block or male block. All persons must individually register for Sikh2Inspire Camp. Please be aware this is only an indication of preference and Sikh2Inspire Camp reserves the right to allocate rooms how it sees fit. I consent to taking part in leisure activities organised as part of Sikh2Inspire Camp 2023
I accept that I will be able to wear a turban to take part in leisure activities and will not be required to wear any items of head protection; and I accept that I will be required to put a sweatband or other suitable covering over my Kara (bracelet) in order to avoid injury whilst taking part in leisure activities
I accept personal liability for any injury (to myself or other persons) resulting from my actions whilst at SIKH 2 INSPIRE CAMP. I accept that I will be reprimanded for failing to follow instructions given by instructors or sevadars at sikh2Inspire Camp
I accept that Sikh2Inspire will be recording/taking photo and video footage throughout the camp and understand that footage and photography may be used for marketing purposes in the future.
I consent to my child taking part in Sikh2Inspire Camp in accordance with arrangements detailed on the application form, web site, supporting literature and as otherwise notified. I accept that alterations may be made to the programme I consent to my child receiving any emergency dental, medical or surgical treatment (including the administration of anaesthetics) that may be advised by a doctor
